Bruno Lowagie, iText Author, give me a tip about "how to use iText in Lotus Notes" in his blog.
There's even an example on how to use iText in Lotus Notes. I would add an extra tip: get the iText code from the 1.3 branch in SVN and compile it with JDK 1.3. That's much better than using iText 1.3.6 and complain on the mailing list about bugs that have been fixed ages ago ;-)
